New Patient and Annual Wellness Visit appointments please arrive 30 minutes before your scheduled appointment.
Same day sick please arrive 15 minutes prior to your scheduled appointment and call office when you arrive in parking lot.
All other appointments please arrive 15 minutes prior to your scheduled appointment.
Be sure to have your insurance cards and photo identification.
